Wellness Checkup Schedule\u00a0<\/span><\/h3>\n

Our clinic performs wellness checkups following the schedule recommended by the American Academy of Pediatrics, listed below.\u00a0<\/span><\/p>\n

 <\/p>\n

A few days after birth, 1 week, 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 2 years old, 2 and \u00bd\u00a0 years old, 3 years old, 4 years old, then once every year afterward.\u00a0<\/span><\/p>\n

 <\/p>\n

If you would like to shift the timing of your child\u2019s wellness checkup, please check with your insurance company to ensure the visit is covered. Some insurance plans may only cover the wellness checkup if it has been at least one year since the last visit, while others may cover it as long as it is a new calendar year.\u00a0<\/span><\/p>\n

**Please keep in mind that you will be charged a cancellation fee if you do not contact us to cancel or reschedule your appointment 24 hours in advance.<\/p>\n

Wellness Checkup Checklist\u00a0<\/span><\/h3>\n

Wellness checkup checklists vary depending on age and are based on the American Academy of Pediatrics recommendations. The urine test is not included in that checklist. If you wish to do a urine test, please tell us when you make an appointment for your child\u2019s wellness checkup.<\/span><\/p>\n

For patients coming in for a wellness checkup as the first visit\u00a0<\/span><\/p>\n

 <\/p>\n

Please submit the following documents using the link \u201csend documents.\u201d <\/span>Alternatively, please send it by fax to 310-564-2356 or bring it to our hospital.<\/span><\/p>\n

\u00a0<\/span><\/p>\n

**After sending all the following documents, please call us to make an appointment. After receiving all the needed documents, an appointment can be placed in about a week or two. In rare cases, we may not receive the documents if too many pictures are attached, so please send them in small batches.\u00a0\u00a0<\/span><\/p>\n

  1. Patient information form<\/span><\/a>\u00a0<\/span><\/li>\n
  2. Statement of Financial Responsibility,etc.<\/span><\/a><\/li>\n
  3. Questionnaire<\/span><\/a>: If no applicable answers are provided, please do not leave the field blank and write \u201cnone.\u201d\u00a0<\/span><\/li>\n
  4. Insurance ID card: Please send both the front and back of the card.\u00a0<\/span><\/li>\n
  5. Immunization record: Please send all immunization records, including out-of-state and out-of-country ones.\u00a0<\/span><\/li>\n
  6. Documents to be submitted to school: Admission documents\/ Sports clearance documents.\u00a0<\/span><\/li>\n
  7. If you have a referral letter.\u00a0<\/span><\/li>\n
  8. Authorization for non-parent\/ guardian to accompany the patient<\/span><\/a>: Our clinic generally only allows parents\/ guardians to accompany the patient. If, for any reason, the parent\/ guardian cannot accompany the patient, please fill out this form and have the authorized person who is accompanying the patient bring it with them when they come to the clinic with an ID.\u00a0<\/span><\/li>\n<\/ol>\n
