Please keep in mind that you will be charged a cancellation fee if you do not contact us to cancel or reschedule your appointment 24 hours in advance.

New Patients

For those coming in for a physical exam for the first time, please bring in a copy of your children’s immunization records before the appointment. You can also send it via fax or through this website. Please understand that it may not be possible for us to newly issue the immunization record specified by the state of California (the Yellow Card) if your children’s immunization records were not sent to us before the appointment. Please give us a call for more details.

If you are bringing your child in for something other than a physical exam or immunization, please bring those records when you come to your appointment.

 

Those who are changing doctors

If you are changing doctors, transferring your medical records from your previous doctor is important. Please fill out this form and fax, mail, or directly hand it to your last doctor’s office or our clinic. The medical records usually take 1-2 weeks to be sent to our clinic. We can see you without the medical records from your previous doctors, but having them will help us provide the best care for you.

 

Expecting mothers

For expecting mothers, we are affiliated with Little Company of Mary in Torrance and Torrance Memorial Center. If you give birth at these hospitals, appointing Dr. Naoko Matsumoto as your newborn’s PCP will allow her to go to the hospital to examine your newborn. This comprehensive care ensures the best start for your baby.
