**We are a fully appointment based clinic. We may be able to take in same day appointments, so please give us a call.

Please be aware that a cancellation fee will be charged if you do not contact us to cancel or reschedule your appointment 24 hours prior.

New Patients

For those coming in for a physical exam for the first time, please bring in a copy of your children’s immunization records before the appointment. You will also be able to send it via fax or through this website. Please understand that it may not be possible for us to newly issue the immunization record specified by the state of California (the Yellow Card) if your children’s immunization records were not sent to us before the appointment. Please give us a call for more details.

If you are bringing your child in for something other than a physical exam or immunization, please bring those records when you come to your appointment.

 

Those who are changing doctors

If you would like to transfer medical records from your previous doctor, please fill this form out and fax, mail, or directly hand it to your previous doctor’s office or our clinic and it will usually take 1-2 weeks for the medical records to be sent to our clinic. We will be able to see you without the medical records from your previous doctors as well.

 

Expecting mothers

Since we are affiliated with Little Company of Mary in Torrance and Torrance Memorial Center, if you give birth at these hospitals, Dr. Naoko Matsumoto will be able to go to the hospital to examine your newborn. In order for us to do this, please appoint Dr. Naoko Matsumoto as your newborn’s PCP.
