Wellness Checkup Schedule 

At our clinic, we perform wellness checkups following the schedule recommended by the American Academy of Pediatrics, listed below. 

 

A few days after birth, 1 week, 1 month, 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 2 years old, 2 and ½  years old, 3 years old, 4 years old, then once every year afterward. 

 

If you would like to shift the timing of your child’s wellness checkup, please check with your insurance company to ensure the visit is covered. Some insurance plans may only cover the wellness checkup if it has been at least one year since the last visit, while others may cover it as long as it is a new calendar year. 

 

Wellness Checkup Checklist 

Wellness checkup checklists vary depending on age and are based on recommendations from the American Academy of Pediatrics. The urine test is not included in that checklist. If you wish to do a urine test, please tell us when you make an appointment for your child’s wellness checkup.

 

For patients coming in for a wellness checkup as the first visit 

 

Please submit the following documents from the link “send documents.” Alternatively, please send it by fax to 310-564-2356 or bring it to our hospital.

 

**After sending all of the following documents, please give us a call to make an appointment. After all the needed documents are received, an appointment can be placed in about a week or two. In rare cases, we may not receive the documents if there are too many pictures attached, so please send them in small batches.  

 

  1. Patient information form 
  2. Statement of Financial Responsibility,etc.
  3. Questionnaire: If there are no applicable answers, please do not leave it blank and write down “none.” 
  4. Insurance ID card: Please send both the front and back of the card. 
  5. Immunization record: Please send all immunization records, including ones issued out-of-state and out-of-country. 
  6. Documents to be submitted to school: Admission documents/ Sports clearance documents. 
  7. If you have a referral letter. 
  8. Authorization for non-parent/ guardian to accompany the patient: In general, our clinic only allows parents/ guardians to accompany the patient. If, for any reason, the parent/ guardian cannot accompany the patient, please fill out this form and have the authorized person that is accompanying the patient bring it with them when they come to the clinic with an ID.

Please bring all of the documents listed above when visiting our clinic. 

 

If you would like for us to issue the California immunization record, also known as the Yellow Card, please inform us when making the appointment.
